Release for Participation in Rush Street Productions Events

In consideration for the opportunity to participate in one or more Poker Night in America® ("PNIA") events, I hereby agree
as follows:
I consent to being recorded and photographed for the PNIA events (including without limitation behind-the-scenes, pre-show,
and post-show photography and recordings). Without notice to me and without further compensation or consideration, I
hereby irrevocably grant to Rush Street Productions, LLC (“RSP”) and its affiliates, and its and their respective officers,
directors, members, managers, employees, agents and all others authorized by them, and all of their respective successors and
assigns (collectively, the “Licensed Parties”), the worldwide, royalty-free, perpetual, sublicensable right to use, modify,
reproduce, display, perform, make derivative works of, animate, distribute, and publish (collectively, “Utilize”) my likeness,
image, voice, personal and distinctive characteristics, and name (and/or a fictitious name) (collectively, my “Likeness”) for
reproduction, distribution, publication, transmission, broadcast, and/or performance of the PNIA event recordings and
photographs in any manner or media, whatsoever, whether now known or hereafter developed, and in any and all products,
services, and/or marketing materials of the Licensed Parties, in any manner or media whatsoever, whether now known or
hereafter developed (including, without limitation, in PNIA-branded games and applications).
I agree and acknowledge that I do not have any ownership interest in the recordings or photographs of the PNIA events that
include my Likeness, and that the Licensed Parties may acquire copyright protection in such recordings and photographs, and
any and all products, services, advertisements, and other materials Utilizing my Likeness. I agree that no product, service
advertisement, or other material Utilizing my Likeness needs to be submitted to me for approval, and that the Licensed
Parties shall be without liability to me for any distortion, retouching, modification or illusionary effect or other adverse result
arising from the Utilization of my Likeness. Nothing contained herein will constitute any obligation on the Licensed Parties
to make any use of the rights set forth herein.
I, on behalf of myself and my heirs, executors, administrators and assigns, hereby fully and forever waive, release, discharge,
and hold harmless the Licensed Parties from any liability, claims, costs, and expenses resulting from or arising out of my
participation in the PNIA events and the Licensed Parties’ Utilization of my Likeness as permitted hereunder, including, but
not limited to, any claims for right of publicity, invasion of privacy, false light, and/or defamation. I, on behalf of myself and
my heirs, executors, administrators and assigns, hereby assume all risks of personal injury, death, property loss, or other
damages that may result from or arise out of my participation in the PNIA events and Utilization of my Likeness.
During my appearance at and participation in PNIA events, I agree to dress as directed by RSP and adhere to RSP's
restrictions on the use of headphones, clothing, and any other items containing endorsements of any kind. I acknowledge and
agree that RSP shall have the sole and exclusive discretion to prohibit the wearing of any item by me at the premises of a
PNIA event, and may immediately disqualify me from the PNIA event for refusal to comply with the directions of RSP in
this regard, without recourse of any kind or nature whatsoever by against RSP. I represent and warrant that I have not
accepted, and will not accept any money, service or other valuable consideration from anyone for endorsing any product or
service at a PNIA event, except to the extent that the same shall be approved by RSP in its sole discretion and in compliance
with U.S. Federal Communications Act requirements.
I shall treat all information and material received or acquired during participation in the PNIA events, including without
limitation, event outcomes, player identities and winnings, operational methods, and production locations, as strictly
confidential and shall not disclose any such information to any third party for a period of three (3) years from the conclusion
of the relevant PNIA event.
This release contains the entire understanding between me and the Licensed Parties regarding the subject matter hereof and
supersedes all prior understandings. No waiver, modification or additions to this release shall be valid unless in writing and
signed by me and an authorized representative of RSP. This release shall be construed in accordance with the substantive
laws of the State of Illinois, without regard to its conflict of law rules.
By signing this release, I attest that I have read and understand all the foregoing provisions of this release as well as the house
rules for the game(s) in which I am participating (which I have received a copy of), prior to its execution, and agree to be
bound by them.
